Partners HealthCare at Home - Home Hospital ProgramWhat Is Home Hospital?PartnersHealthCare at Home works with MGH and is based on our patients strongdesire to maximize their time at home. When illness develops and is significantenough to require a hospital stay, Home Hospital is able to provide similarcare for patients at home to avoid time in the hospital. You can think of HomeHospital as sending the hospital home with the patient. Home Hospital is moreadvanced than traditional home health care. Just like if a patient were in the hospital, thisprogram would provide for several visits with our clinicians each day totreat the conditions until the patient is safe enough to be cared for by aregular clinical team.Commonconditions treated by Home Hospital include congestive heart failure, COPD,pneumonia, skin infections such as cellulitis and urinary tract infections aswell as many other conditions deemed safe enough to do in the home.Inthis role the RN supports the Home Hospital Program based at MGH and worksin a 10 mile radius surrounding the hospital treating patients in that areaJob SummaryProvidequality, clinical services to patients in various settings in thecommunity. Patient care is deliveredwithin organizational philosophy, policy and standards of nursing and communityhealth practice. The responsibilitiesare performed in accordance with PHH policies and standards of practice, utilizingthorough and timely electronic documentation and interdisciplinarycommunications. Skilled nursing care isprovided in the form of assessment, teaching, treatment, documentation and carecoordination demonstrating high quality customer service, and financialawareness.Qualificationsand Experience* Graduate of an approved School of Nursing* Current Massachusetts license as Registered Nurse* Minimum of one year previous nursing experience in acute, emergency or post-acute care setting required. * Prior home care experience preferred.* Travel required within PHH's geographic area* Ability to work with various computer software applications and technologies required.* Strong interpersonal and customer service and ability to work with a diverse population* Ability to communicate effectively in writing, verbally and electronically* Ability to work as a member of an interdisciplinary team of health care providers* Ability to organize and prioritize work, and adapt to changing situations* Ability to work independently, be self-directed and adapt to unpredictable circumstances.EEO StatementPHH is committedto diversity in the workplace which begins with respect and opportunity forall.
