Location: -CHA Everett Hospital -Work Days: - 7:00am-3:30pm & 3:00pm-11:30pm, Rotating Weekends and Holidays. - -Category: -Registered Nurse - -Department: -Psych EH Lewis 2 Nursing - -Job Type: Full time - -Work Shift: Day / Evenings -Hours/Week: 40.00 -Union: Yes -Union Name: MNA EverettDEPARTMENT DESCRIPTION:Lewis 2 is a 22 bed Geri Psych/Behavioral Health Unit for patient population 55+. We provide a vibrant activities program along with special group sessions and programming to provide support. A special group room houses musical instruments, art supplies and other items to engage individual patient strengths and interests. Patients are cared for by an entire team consisting of: Psychiatrists, Registered Nurses, Social Workers, Case Managers, Milieu Therapists and Nursing Assistants. - -Summary:
The Registered Nurse is responsible and accountable for planning and providing patient care for patients in accordance with established Nursing Standards of Care. He/she demonstrates initiative, knowledge and clinical skills in caring for the patient with complex needs. The Registered Nurse demonstrates the ability to effectively manage patients by assuming full responsibility for the assessment, plan, implementation and evaluation of nursing care and is directly reports to a designated nurse manager and/or clinical manager / off shift manager.Responsibilities:
--- Identifies own strengths, limitations, and values in functioning as a member
--- Elicits others advice and perspectives including that of the patient and participates fully in interdisciplinary forums that promote an integrated approach to patient care, e.g. multidisciplinary rounds, team meetings
--- Access appropriate resources to advocate for and optimize patient care considers the interests of all team members, making choices that best benefit patient family Advocacy and Moral Agency
--- Understands the concepts of accountability for own nursing practice able to recognize basic ethical
--- Recognizes tensions between professional autonomy and systems factors demonstrates professional comportment behaviors including attention to appearance demeanor, respect for self and others and attention to professional boundaries among patients, families, and caregivers
--- Recognizes the need for using an ethical decision making framework for clinical situations and obtains assistance in solving ethical dilemmas as needed
Caring Practices:
--- Expresses empathy and compassion for patients and families treats everyone with courtesy, dignity and respect consistently participates in patient rounding/ huddle/ use of white boards/ bedside
--- Demonstrates emotional intelligence, i.e., the capacity to perceive, assess, and manage the emotions of one's self and others

--- Provides patient centered care with sensitivity and respect for the diversity of human experience

--- Sets plan of care priorities based on identified patient needs and uses appropriate technologies in the process of assessing and monitoring patients and documenting care collaborates with interdisciplinary team to plan for multiple complex patient care

--- Follows evidence based standards and guidelines
--- Recognizes changing patient conditions and responds immediately
--- Describes reliable sources for locating research, evidence reports and clinical guidelines relevant to clinical practice area
--- Tailors standards and guidelines to the needs of individual patients
--- Uses QI tools and methodologies to assess performance and identify gaps between current unit & best practice related to key clinical outcome measures
Systems Thinking:
--- Considers system resources available in the practice area when developing the plan of care for a patient or group of patients
--- Brings issues of concern at the practice area level to the attention of others when assistance is needed to facilitate resolution
--- Demonstrates understanding that care and services are delivered in a variety of settings along a care continuum
--- Recognizes how to obtain resources beyond those of the unit and does so on behalf of patients/families as needed Identifies and works to remove barriers that block the delivery of reliable customer service
Facilitation of Learning:
--- Initiates independent learning based on identified needs and reflective practices
--- Actively engages as a "super user" or "unit champion" during roll out of new products equipment, processes, e.g., EPIC, rounding, code chill, etc .

--- Based on learning needs assessment, effectively uses teach back methodology, motivational interviewing, and self-management goals to provide meaningful patient/family education

--- Participates in activities that help formulate solutions to unit ineffectiveness and/or failures that impact patient care, e.g., recommends change in policies and procedures through appropriate

--- Demonstrates engagement in lifelong learning and self-assessment of ongoing professional development needs

--- Is an effective steward of available clinical resources, e.g., accurate assessment of patient acuity when making decisions regarding staffing, transfers and assignments
Customer Service:
--- Creates positive impressions that consistently demonstrate actions associated with CHA service standards and professionalism
--- Consistently "manages up" other co-workers, departments, providers, and
Patient Safety:
--- Adheres to established safety practices, e.g., National Patient Safety goals and evidence based guidelines / protocols to prevent adverse patient outcomes
--- Adheres to regulatory requirements, e.g., DPH, TJC, DMH, Medicare, etc
--- Adheres to the five rights of medication administration, and related documentation standards
--- Adheres to department documentation standards and uses data from electronic record to guide patient care
--- Uses available technologies to support and enhance patient care, e.g., smart pumps, call bell system, EPIC reports, smart phrases, acuity tools, etc
Technical and Procedural Skills
--- Demonstrates appropriate procedural /technical competence specific to practice area ( Unit / Clinic Specific Competencies & required certifications and work experience)
BSN preferred
Licensure: MA. RN Licensure required
Certifications: Maintains current Basic Life Support "C" certification
AHA or ARC BLS Healthcare Provider
