Case Manager RN Job DescriptionMass General Brigham is seeking a skilled Registered Nurse (RN) to join our Case Management team in Chelsea, Massachusetts.Job SummaryWe are looking for a dedicated and compassionate RN to work as a Case Manager, collaborating with the healthcare team to enhance patient care services along the continuum of care. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work effectively in a fast-paced environment.Responsibilities

• Evaluate, predict, and facilitate the trajectory of patient care
• Employ a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation to meet individual health care needs
• Collaborate with the healthcare team to ensure high-quality patient care, reduce costs, and ensure reimbursement
• Participate in the development and implementation of patient care plans
• Monitor and evaluate patient outcomes, making adjustments to care plans as neededRequirements
  • Current RN licensure in the state of Massachusetts
  • Minimum of 2 years Med/Surg experience
  • Case Manager certificate preferred
  • ADN required, but BSN strongly preferred
