struggling with substance use disorders (SUD) Description: The
Registered Nurse is responsible for providing nursing care to
patients. The RN supervises care treatment and services provided by
LPNs, CNAs, and other staff on the assigned unit and shift. The RN
develops, implements, and evaluates an individual plan of care for
each patient. Assessment and re-assessment of patients are
essential tasks of this role. The RN communicates appropriate
information with the interdisciplinary team. Patient concerns and
changes in condition must be reported to the supervisor and/or
physician in a timely manner. The RN provides and coordinates care
by assessing the physical and behavioral health needs of patients.
They develop and implement nursing care plans, maintain medical
records, and educate patients and their families about various
physical and behavioral health conditions while ensuring patient
safety at all times. Ongoing assessment of the patient’s status is
required, and pertinent information gathered must be documented.
Nursing treatment plans should be specific to assessed needs, and
the initial treatment plan must be completed upon admission. The RN
takes physician’s orders accurately and documents them
appropriately, transcribing and reviewing physician orders as
necessary. Legibility of physician orders is ensured, especially if
taken verbally. The RN accurately administers medications as
ordered and documents the accurate count of controlled substances.
They adjust medications under provider supervision as needed and
monitor lab results to validate medication compliance. Conducting
psychoeducation and life skills groups is part of the RN’s
responsibilities. The RN notifies the physician of significant
changes in patient status, such as difficulties with detox, medical
emergencies, or changes regarding response to medications.
Supervision of self-administration of medications is necessary,
ensuring the use of two means of patient identification.
Documentation and reporting of effects of scheduled and PRN
medications are essential. The RN efficiently completes admission
and discharge processes as directed. They confer regularly with the
treatment team regarding patient care. Participation in unit
activities such as therapy groups, family meetings, staffing, and
unit meetings is required. Education is provided to patients,
families, and co-workers regarding medication usage and
maintenance, common side effects, and patient rights. Monitoring
patient acuity, census, staffing needs, and making arrangements for
coverage as needed is part of the role. Thorough and accurate
documentation for all patients on each shift is essential. Daily
patient assignment sheets must be completed, and all tasks carried
out as assigned. The RN gives thorough reports to oncoming staff,
supervisors, and physicians. Providing coverage in emergency
services as needed is also part of the responsibilities.
