Job Title: RN Case Manager Profession: Registered Nurse Specialty: Home Health Duration: 13 weeks Shift: Days Hours per Shift: 36 Experience: Minimum 2 years of home care experience License: Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license Certifications: Graduate of an accredited school of nursing Must-Have: - Two years of home care experience - Licensed in Vermont and New Hampshire Description: RN Case Manager for a full-time position. You will have the opportunity to know your patients deeply and become an integral part of their care team. You will be provided with broad independence while being part of a cohesive patient-care team. This role offers a work/life balance that can match the flexibility you need. As an RN Case Manager, you will screen, evaluate, re-evaluate, and treat patients. You will coordinate and communicate treatment activities with interdisciplinary team members and patients. You will develop a care plan in collaboration with the patient's physician, the patient, and their family or primary caregivers upon admission. You will periodically review and recertify the patient care plan as appropriate. You will coordinate the patient’s care plan through interdisciplinary communication and case conferences as necessary. You are expected to advise other disciplines of discharges in a timely manner. You will provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals. You will communicate significant changes to the patient's physician and colleagues. You will document evaluations, treatment goals, and objectives. You will teach the patient and caregiver self-care techniques as appropriate. You will provide medication, diet, and other instructions as ordered by the physician. You will utilize opportunities for health counseling with patients and caregivers.
