Health & Hospice Registered Nurse – Vermont Start Date: February 2,
2026 Location: Newport, Vermont Compensation: Details provided to
qualified applicants Schedule: Monday through Friday, 8:00 AM –
5:00 PM, with some weekend coverage required. Call may be scheduled
on an emergency basis only. Highlights: Deliver compassionate
outpatient home health and hospice care directly in patients' homes
Utilize HealthWyse (Careficient) EMR system for streamlined
documentation Manage a dynamic schedule with travel between patient
visits throughout the community OASIS assessment experience
essential for comprehensive patient evaluation Primarily weekday
schedule with minimal on-call responsibilities Requirements: Active
Vermont RN license in good standing OASIS (Outcome and Assessment
Information Set) experience required Home Health and Hospice
nursing experience required Valid driver's license in good standing
with reliable transportation Current BLS certification Ability to
manage and adapt to schedule changes effectively Must pass facility
