working with Lancesoft Inc to find a qualified Case Manager RN in
Lebanon, New Hampshire, 03756! Job description: Position Overview:
Title: RN – Case Manager Location: Lebanon, NH Duration: 13 weeks
with possible extension Shift: Day shift, 8 hours (8:00 AM – 4:40
PM, Monday–Friday) Hours per week: 40 On Call: No Role
Responsibilities: Oversee a daily caseload of patients on inpatient
units with diverse care needs. Attend interdisciplinary rounds and
complete initial assessments. Develop care plans collaboratively
with patients, families, caregivers, and healthcare teams.
Coordinate post-discharge needs (home health, inpatient rehab, DME,
nutrition). Communicate consistently with medical teams and
patients about progress and discharge planning. Place orders for
home health, durable medical equipment, and nutritional needs in
EPIC EMR . Ensure smooth transitions of care and effective resource
utilization. Requirements: License: Registered Nurse (NH or Compact
RN License) Education: BSN required Experience: 3 years inpatient
clinical experience At least 1 year Care Management experience
