Job Description:

Massachusetts General Hospital has a four-part mission including patient care, teaching, research and "improving the health and well-being of the diverse communities we serve." For almost 25 years, the Mass General Center for Community Health Improvement (CCHI) has engaged in multiple partnerships with under-resourced communities to address community-identified public health issues, such as opioids and access to healthy foods. CCHI conducts community health needs assessments and develops implementation plans with communities every three years.

Mass General Center for Community Health Improvement (CCHI), the MGH Division of General Internal Medicine, and the MGH Division of Primary Care Pediatrics have launched a multi-stakeholder effort, with the goal of ensuring that screening for Social Determinants of Health (SDOH) and responding to identified needs are part of standard care in Primary Care. An additional goal of this effort is to develop and evaluate programming to decrease disparities in chronic disease and prevention outcomes through intensive community health worker interventions.

Under the direction of the DGIM Senior Administrative Manager and the Primary Care United Against Racism (UAR) Physician Leader and under the direct supervision of the Director of Community Health Improvement, the project manager will assist primary care UAR teams and stakeholders in accomplishing their goals/objectives. This is a 40 hour per week position and will be located within the offices of the MGH Center for Community Health Improvement to support a dedicated and ambitious plan to improve health and ahieve health equity in pirmary care.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Provide excellent project management service to support a comprehensive, multi-phased effort to adopt universal SDOH screening and response, and chronic disease navigation to address disparities in outcomes. Under the direction of the Senior Administrative Manager, the project manager will work closely with subcommittee co-chairs to:
  • Assist in the design and execution of planning activities required to implement key priorities.
  • Develop schedules/timelines, coordinate activities, track project status and measures of success.
  • Plan, prepare and organize meetings. Create meeting agendas, record minutes and follow-up with team to coordinate next steps.

  • Research relevant issues and collect/synthesize and/or manipulate relevant data (ex. create graphs, etc.) using multiple sources.
  • Monitor and communicate progress, as well as disseminate results of projects.
  • Organize performance data to submit for review
  • Help develop and distribute materials (e.g. PowerPoint presentations, spreadsheets, reports, etc.).
  • Ensure smooth information flow between involved parties.
  • Prepare summary communication materials for leadership and stakeholder presentations, as requested.
  • Maintain all project documents, data, action items and other key information.
  • Work with Epic to track IT enhancements to facilitate the work of the team.

  • Some support work such as scheduling meetings and maintaining distribution lists will be required.
  • Collaborate & communicate across stakeholder groups within CCHI, Primary Care, PHC Teams & MGB
  • Other duties as assigned.

SKILLS & COMPETENCIES REQUIRED:

  • Strong time management, organizational and planning skills and the ability to multitask at a higher level.
  • Excellent verbal, written and interpersonal communication skills.
  • Great attention to detail, professionalism and a strong work ethic.
  • Demonstrated high degree of initiative with ability to prioritize work, meet deadlines and manage multiple projects simultaneously.
  • Demonstrated ability to be flexible and adaptable in a fast-paced complex environment.
  • Ability to work independently which includes good judgement about when to seek support.
  • Ability to work in a collaborative team environment and effectively interact with all levels of MGH.
  • Experience facilitating and staffing projects that involve interdisciplinary team members a plus.
  • Proficient in MS Office applications, including Outlook, Word, Excel and PowerPoint.
  • Experience with collecting, summarizing and presenting data a plus.
  • Passion for/experience with community health improvement, achieving health equity and addressing social determinants of health preferred.

EXPERIENCE:
