Job Description:
NSMHA is looking for Family Partners to join our Community
Service Agency! As a Family Partner, you will provide a structured
one-to-one, strength-based relationship to a parent/caregiver of a
Seriously Emotionally Disturbed youth for the purpose of addressing
the youth's emotional and behavioral needs. You will work with
parents and caregivers to enhance parenting capacity and increase
access to natural community resources to enable the
parents/caregiver to parent the youth successfully.
This position is designed for someone with lived experience as a
parent or direct caregiver of a youth in their family with special
needs (preferably mental health needs).
This position is currently a hybrid position: community visits and
remote work, contingent on family preference.
What you'll do:
- Engage the parent/caregiver in activities in the home and
community.
- Assist the parent/caregiver in addressing one or more goals on
the youth's treatment plan for Outpatient or In-Home Therapy or
Individual Care Plan.
- Coordinate Intensive Care Coordination Services with the Care
Coordinator within the agency and Hub providers outside of the
agency.
- Assist the parent/caregiver in meeting the youth's needs by
educating, supporting, coaching, modeling or guiding and fosters
empowerment.
- Deliver services in the parent/caregiver's home and
community.
- Participate, attend and support parent/caregiver and Care
Coordinator's for Individual Care Plan meetings.
- Maintain regular contact with the parent/caregiver
(face-to-face and telephone).
- Provide family education, advocacy and support to youth and
family.
- Monitor, review and update the ICP to reflect the changing
needs of the youth and family.
- Adhere to policies and procedures governing client rights and
confidentiality.
- Complete appropriate documentation of service delivery in a
timely manner. What you'll need:
- Education
- BA in a Human Services field and one year of experience OR
- Associates degree in a human service field and one year of
related experience OR
- High School diploma/GED and a minimum of two years relevant
experience.
- Experience:
- Lived experience as a parent or direct caregiver of a youth in
their family with special needs (preferably mental health needs)
required.
- Experience working with parent/caregivers and youth with
Serious Emotional Disturbance.
- Experience navigating any of the child and family serving
systems.
- Familiarity with child & youth serving programs
- Skills:
- Bilingual Spanish preferred (please indicate language skills on
application)
- Valid Mass drivers license and offsite travel required.
- Vaccination:
- At least one shot of the COVID-19 vaccination is required prior
